Palliative care has taken place in the health system in the world over the last 30 years as a care system that includes medical and supportive treatments to alleviate the suffering of persons with difficult or impossible treatment (1). It is not only maintenance done in the last period of life but it is a form of treatment that should be integrated in the treatment for prolonging the life span regardless of the disease stage (2). According to the definition of the World Health Organization, 'the approach aims to early identification, evaluation and treatment of pain and other physical and psychological problems that may occur in people with serious illness, and to increase the quality of life in people with lifethreatening diseases and their families (3). Palliative care can be provided at the ward in the outpatient clinic, outpatient specialized support homes and home care teams at home, with a dedicated inpatient service and / or consultation team. Depending on the symptom load and the nature and complexity of the support, the maintenance is ensured by switching between these spaces (4). General patient services may not be sufficient for symptom management in cancer patients. Therefore, the establishment of inpatient palliative care services will enable effective service delivery. Palliative care units are also important because they are an application that reduces resource use and hospital costs (5). In this study, we aimed to evaluate retrospectively the patients treated and followed-up in our hospital palliative care service.

In this study it was aimed that as a non invasive technique, quantitative insulin sensitivity check index-(QUICKI) levels which has reflected the insulin sensitivity status, were compared in women with PCOS and regular menses. Additionally, we evaluated whether this index may have been used in clinical practice of PCOS.

17-year-old epileptic patient with moderate mental retardation was taken to intensive care unit from emergency service as intubated due to fall from height. The unconscious patient was intubated, Glasgow Coma Scale (GCS) 4, tension arterial 100/65 mmHg, hearth rate 105 bpm, peripheral oxygen saturation 100, mechanically ventilated in SIMV mode with respiratory rate 14, tidal volume 400 ml. In patient’s CT and MRI, areas that comply with axonal injury were observed (Figure 1), steroid and mannitol for brain oedema treatment was applied. The patient was extubated on the 15th day after his sufficient spontaneous ventilation. Percutaneous endoscopic gastrostomy (PEG) was applied due to lack of oral feeding ability. Because of extended pneumonia treatment in intensive care unit, the patient was taken to palliative care unit on 45th day of his hospitalization. The patient had tetraplegia, lung infection, and no ability to communicate. Medical treatment was planned. The patient’s relatives were told to communicate with him, as if he would understand them. In 10 days, the patient started to make eye contact and recognize his doctor. Once the patient was observed to regain swallowing reflex, he was fed orally, on 50th days of hospitalization PEG was removed. Increase in motor strength starting from lower extremity towards upper extremity was observed, muscle strengthening exercises were practiced by physical therapy program. The patient was discharged from palliative care on his 75th day as he recovered and gained his physical and psychological condition before the event.

Echinococcus granulosus is a parasitic disease and the causative agent of cyst hydatid disease which is common in our country and commonly found in nature. Even though it can hold onto any tissue of body, most commonly it holds onto the liver (1-3). Hydatid cyst is still one of the major health problems in our country. Its frequency ratio is 2-6 in 100,000 and it is placed in the liver at 60-70% ratio and in the lung at 20-25% ratio (4). Diaphragmatic localization is at a rate as low as 1% and is usually associated with liver hydatid cyst. In such localization of a cyst, is not always possible for the location of the cyst to be fully determined by radiological methods (5,6). In this paper, a liver cyst hydatid case causing intraoperative diaphragmatic rupture because of its proximity to the diaphragm is presented. Case Report
