A LOW-COST AND LIGHTWEIGHT 3D INTERACTIVE REAL ESTATE-PURPOSED INDOOR VIRTUAL REALITY APPLICATION

Kasım Özacar · Yasin Ortakcı · İdris Kahraman +2 more

Interactive 3D architectural indoor design have been more popular after it benefited from Virtual Reality (VR) technologies. VR brings computer-generated 3D content to real life scale and enable users to observe immersive indoor environments so that users can directly modify it. This opportunity enables buyers to purchase a property off-the-plan cheaper through virtual models. Instead of showing property through 2D plan or renders, this visualized interior architecture of an on-sale unbuilt property is demonstrated beforehand so that the investors have an impression as if they were in the physical building. However, current applications either use highly resource consuming software, or are non-interactive, or requires specialist to create such environments. In this study, we have created a real-estate purposed low-cost high quality fully interactive VR application that provides a realistic interior architecture of the property by using free and lightweight software: Sweet Home 3D and Unity. A preliminary study showed that participants generally liked proposed real estate-purposed VR application, and it satisfied the expectation of the property buyers

Re-expansion Pulmonary Edema Caused by Pleural Effusion Drainage in Late Period After Coronary Artery Bypass Surgery: Case Report

Kadir Kaan Özsin · Faruk Toktaş · Umut Serhat Sanrı +2 more

Reexpansion pulmonary edema usually follows rapid reinflation of lung parenchyma which collapsed more than three days and can be fatal in up to 21% of cases. A 48-year-old man who had undergone CABG (Coronary Artery Bypass Graft) three weeks ago, admitted to our hospital with a diagnosis of left pleural effusion. He had the clinical findings of pulmonary edema, respiratory insufficiency and shock after tube thoracostomy. He was mechanically ventilated in intensive care unit. During this period, fluid restriction, inotropic agent, diuretic and oxygen support therapy were applied. The patient discharged on 7th day with a full clinical and radiological recovery. The aim of this case report was present to attention the development of re-expansion pulmonary edema which may be fatal, after tube thoracostomy in a patient with pleural effusion following coronary artery bypass surgery.

Evaluation of Shunt Use with Near Infrared Spectroscopy During Carotid Endarterectomy: An Observational Clinical Study

Kürşad Öz · Salih Güler · Zeynep Kahraman +2 more

Objective: Measurement of cerebral saturation (NIRS) with is that noninvasive method can early and easily demonstrate disorders in cerebral blood flow without the need for specialized personnel. In this study, we evaluated not only the relationship between cerebral monitoring values and shunt use but also postoperative short term effect of cerebral monitoring in patients undergoing carotid endarterectomy. Material and Methods: Between 2010 and 2015, 427 patients underwent 481 carotid endarterectomy operations. Sixty-five of these patients (13.5%) had bilateral critical carotid stenosis. Carotid endarterectomy was performed under shunt requirement in 54 cases (11.2%). The NIRS probe was placed in the frontal region of the patient to monitor the cerebral oxygen level of the patients. At this time, NIRS values of patients were recorded before cross-clamp (rSO 0), cross-clamp (rSO1) and post-clamp (rSO2). Following operation, the patients were transfered to intensive care unit and then to the ward after staying 2-4 hours. Operational time, cross-clamp time, complications within 30 days associated with operation were recorded. Subsequent checks were made by the neurologist on the 30th day and every 6 months on follow-up. Results: The mean age was 69.95±12.84 in group 1 and 66.18±7.11 years in group 2. There was a significant difference in the carotid clamping time (min), percentage of carotid angioplasty, bilateral carotid procedures between group 1 and group 2 (23.85±12.4 vs 15.18±7.11, 100% vs 79.2%, 74.07 vs 5.85, p<0.01, respectively). The length of intensive care unit and hospital stays were similar among groups. regional cerebral oxygen saturation in first 3 min after carotid artery clamping (rSO1) in shunt group was lower compared to the patients in no-shunt group (48.7±11.5 vs 59.7±6.77, p=0.03) Conclusions: As a result, NIRS is a safely and a reliable cerebral monitoring technique and also it may help to reduce unnecessary shunt use in patients undergoing carotid endarterectomy.

Is it Logical Excising All of the Postmenopausal Polyps?

Ali GEMİCİ · Fırat Tülek · Alper Kahraman +3 more

ABS TRACT Objective: The aim of the present study was to address the question whether histologic evaluation is always necessary for women with a suspicion of an endometrial polyp at the postmenopausal period. Material and Methods: Between August 2007 and February 2014 a total of three hundred twenty seven postmenopausal women with a suspicion of endometrial polyp at the trans-vaginal ultrasound with or without a complaint of uterine bleeding were included to the study. Data were collected retrospectively from clinic records. All of the patients treated with hysteroscopy. Pathologic results were the main outcome measure. Results: Two cases of 158 asymptomatic polyps, 16 cases of 169 symptomatic polyps were malign and premalignant. The relative risk of symptomatic polyps is 1,79. The complication rate was 3,05% for all cases. At the symptomatic group 2 cases and at the asymptomatic group 8 cases have a complication. The complication rate was increased nearly by 4 times (p=0,027). Conclusion: Management strategies for postmenopausal endometrial polyps are controversial. In the light of our data, we suggest symptom-based interpretation for choosing management options. We believe that the management choice of an asymptomatic postmenopausal polyp is expectant instead of excision. Nevertheless, further work needs to be done more broadly to establish expectant management alternatives about the asymptomatic postmenopausal endometrial polyps.
