Classifications particularly based on the type of vascular injury are still in use today.1 In absence of an avulsion and presence of adequate blood supply to the finger, primary repair of the soft tissues may be performed, whereas vascular grafting may be necessary in case of arterial or venous circulation failure. The main reason here lies behind the presence of a bigger injury than it looks and the occurrence of vascular injury on a longer segment of the vein. Another repair in addition to the arterial and venous repair in avulsion injuries that requires grafting is the skin repair. This, in turn, additionally creates morbidity in the donor site. An accompanying severe soft tissue damage (avulsion or degloving) or amputation may present a challenge for the reimplantation technique, however, is also directly associated with the rate of success.2 While avulsion injuries are usually experienced in the work place, primary dermabrasion injuries or those which occur as a result of removing a ring from an edematous finger are encountered at home. Odontomas are the most common benign tumors of odontogenic origin. Due to the absence of symptoms, they are usually diagnosed on routine radiographs. Odontomas are divided based on their radiographic, clinical and histological features; compound odontoma and complex odontoma. The region can be reconstructed according to the destruction area of these lesions. Large bone defect in the jaw bone is usually reconstructed with autogenous bone graft from extraoral donor sites. In such cases, anterior iliac grafts are preferred most frequently. We reported 32-yearold man was referred to the Department of Oral and Maxillofacial Surgery, asymmetry with the facial swelling in the region of the posterior right maxilla. The first diagnostic hypothesis was complex odontoma. In decision of surgical excision of the lesion, followed by the rehabilitation of the zone with dental implants. The resulting defects were enriched with anterior iliac crest bone graft.

Castleman disease (CD) is a rare benign lymphoproliferative disorder of unknown etiology, typically presenting as a solitary mediastinal mass. The head and neck region is the most common site; however, the salivary glands are rarely affected. We present a rare case of a 59-yearold male patient with mixed type CD localized in the parotid gland. The patient underwent a left superficial parotidectomy with a clinical prediagnosis of mucoepidermoid carcinoma. H&E stained sections showed mostly preserved lymph node architecture. Some lymphoid follicles had atrophic germinal centers and mantle zones was expanded. Also there were sclerotic blood vessels radially penetrated the germinal centers and the overall appearance of the follicle was resembling a lollipop. The diagnosis was CD, mixed type. For the accurate diagnosis of this entity, histopathological examination should be performed and physicians should kept in mind this clinical entity so patients can receive adequate and early treatment.

Objective: As well as being a risk factor for vascular diseases, hyperhomocysteinemia is also a risk factor for neurodegeneration. The present study examines the effect of hyperhomocysteinemia on cognitive and motor performance in patients with Parkinson’s disease (PD). Material and Methods: 46 PD patients monitored by us and 30 volunteers without a cronic disease to cause hyperhomocysteinemia compatible with this population were included in the study. Blood levels of folate, vitamin B12 and homocysteine of both groups were measured and compared. In the patient group, Standardized Mini Mental Test (SMMT) and The Unified Parkinson’s Disease Rating Scale (UPDRS) scores were determined and serum homocysteine levels were measured to see if there was a correlation between these two. Furthermore, the effect of levodopa intake on blood vitamin B12, folate and homocysteine levels in patients was examined. The assumed significance level is p <0.05. Results: Vitamin B12 and folate levels were significantly lower in the patient group than in the control group, whereas homocysteine levels were significantly higher in the patient group. Levodopa intake had no significant effect on the examined levels. Elevated homocysteine levels resulted in lower SMMT scores and higher UPDRS scores. Conclusion: Many other studies have found hyperhomocysteinemia in PD patients and indicated that hyperhomocysteinemia was one of the causes of decreased cognitive and motor performance during the course of disease. With its finding of decreased performance in the patients, the present study also supports the above mentioned studies and recommends B12 and folate supplements in order to slow down the disease progression.

Objective: The aim of this study was, to examine the presence of Aeromonas salmonicida in lake water and fish species living in Lake Sazlıdere, a region where fish hunting was also conducted, in Istanbul. Material and Methods: In this study, between April 2014 and March 2015, samples from Lake Sazlıdere were collected for once a month. Samples were divided into three groups including, swabs materials from fish surface (n=48), viscera (n=46) and lake water (n=60). The samples inoculated on to the Furunculosis Agar and Tryptic Soy Agar including Coomassie Brilliant Blue (C-TSA) and incubated at 25°C for 7 days. Presumptive colonies were examined with biochemical tests for the identification of A. salmonicida. Concurrently with bacteriological examination, all of the samples were examined for the presence of vapA gene with Polymerase Chain Reaction (PCR). Results: According to bacteriological examination results, A. salmonicida were not isolated from any of the samples. As a result of PCR, of 60 water samples 3 (5.0%) were positive for vapA gene, while A. salmonicida specific DNA was not detected in the viscere and swab samples. Conclusion: It has been determined that infections caused by A. salmonicida, especially furunculosis disease, under this light do not constitute a risk in Lake Sazlıdere at the time of the study. However, the information obtained from fishermen and the results of PCR, when evaluated together with chemical contamination in general, indicate that continual investigations should be continued in this area, not within a certain period of time.
