Is there a relationship between mean platelet volume and the severity of coronary ectasia?
Mean platelet volume (MPV) is an indicator of platelet activation that is elevated in patients with coronary artery ectasia (CAE). We aimed to research relationship between MPV and the severity of coronary ectasia. The reports of 6,377 patients who underwent elective coronary angiography screening between January 2011 and January 2015 were reviewed. After exclusion, 117 patients with isolated CAE were included in this study. The severity of CAE was divided into four types according to the Markis classification. There was no significant difference in age, sex, hypertension, diabetes mellitus, smoking, or family history between the CAE and normal angiogram groups (P>0.05). MPV was significantly higher in patients with CAE than the control group (11.14 ± 1.13 vs. 9.82 ± 0.90 fL, P 10.85 fL may indicate the presence of CAE.
Total pulmonary vein Diameter is strong predictor of atrial fibrillation after coronary artery bypass graft surgery
OBJECTIVERecent studies have shown that the pulmonary veins are important in atrial fibrillation (AF). This study evaluated the relationship between total pulmonary vein diameter and postoperative AF in on-pump coronary artery bypass graft (CABG) patients.PATIENTS AND METHODSOur study enrolled 149 patients undergoing on-pump CABG. The primary endpoint was defined as postoperative new-onset in-hospital AF. All patients underwent preoperative non-contrast tomography to measure pulmonary vein diameter.RESULTSThe patients who developed AF had significantly greater total pulmonary vein diameters than those who remained in sinus rhythm. Logistic multivariate regression analysis revealed that only total pulmonary vein diameter was an independent predictor of the development of new-onset AF.CONCLUSIONSTo our knowledge, this is the first report of an association between total pulmonary vein diameter and the development postoperative AF. The identification of high-risk patients using pulmonary vein diameters should facilitate preventive measures.

Giant Meckel’s diverticulum torsion that mimics adnexal pathology
Meckel's diverticulum is a real diverticulum located at the antimesenteric portion of intestinal loops and including all layers of the intestinal wall. It is the most common congenital anomaly of the gastrointestinal tract, and its incidence is 1-3%. Many asymptomatic cases are diagnosed when complications occur. A 23-year-old female patient applied to gynaecology emergency clinic with pelvic pain complaint. Laparotomy was performed with the diagnosis of acute abdomen because the physical examination and imaging studies did not exclude tuboovary pathology. Giant Meckel's diverticulitis and ischemic bowel loops that had been torsion were observed. Obstruction is the most common complication and generally originates from inflammation, adhesions, intussusception and omphalo-mesenteric band. In this case, it was seen that mobilized diverticulitis can be complicated without any fibrous band or adhesion to adjacent organs. This case supports that there can be torsion of bowel in free Meckel's diverticulum. Meckel's diverticulum settled in the pelvic region can make a clinical manifestation that is difficult to distinguish from adnexal diseases. It should be kept in mind for cases that start with pelvic pain, form adnexal pathology suspicion and cause an acute abdomen.
