Outcomes of Facial Nerve in Lateral and Inferior Mastoid Approaches for Jugular Foramen and Petrous Apex Lesions

Objectives To evaluate the facial nerve in inferior and lateral (transmastoid) approaches to the jugular foramen and/or petrous apex. Design Retrospective study of 11 consecutive patients operated for lesions in the jugular foramen/petrous apex via an inferior or lateral transpetrosal approach. Setting Tertiary care superspecialty referral center. Participants Eleven consecutive patients of jugular foramen/petrous apex lesions operated over a period of 18 months. Main Outcomes Measures Extent of tumor resection, surgical technique used, and recovery of facial nerve function postoperatively. Conclusion Approach to these lesions needs to be tailored to the extent of disease, possible histology and relation to the facial nerve. Meticulous surgery and adequate postoperative care permit a satisfactory recovery of facial nerve function following rerouting procedures.

Vasoactive Inotropic Score and Outcome Assessment in Cyanotic Infants after Cardiovascular Surgery

Objective Prospective assessment of vasoactive inotropic score (VIS) for predicting postoperative outcomes in cyanotic infants following open-heart surgery. Methods In this prospective observational study, postoperative outcomes were analyzed in 100 cyanotic infants operated between September 2014 and April 2016. Inotropic score and VIS were calculated at 1 hour, 24 hours, and 48 hours following surgery. Results Mean age at surgery was 7.39 ± 4.4 months (3 days–12 months, median 9 months). Surgical procedures included repair of total anomalous pulmonary venous connection (n = 40), tetralogy of Fallot (n = 24), double outlet right ventricle with pulmonary stenosis (n = 4), bidirectional superior cavopulmonary anastomosis (n = 19), and miscellaneous (n = 13). On regression analysis, prolonged intensive care unit and hospital stay correlated best with VIS at 48-hour interval (R 2 = 0.67 and 0.50 respectively, p > 0.001). Low birth weight ( 25) were predictive of higher postoperative mortality. Maximum VIS at 24 and 48 hours were positively correlated with adverse postoperative outcomes (p Conclusion In cyanotic infants undergoing open-heart surgery, higher 48-hour VIS was associated with poor postoperative outcome indicating its utility as a valuable tool in predicting postoperative outcomes.

Aortomegaly Causing Nutcracker Syndrome and Pelvic Congestion Syndrome

The authors present an uncommon cause of pelvic congestion syndrome (PCS) secondary to anterior nutcracker syndrome, which was caused by aortomegaly. Positional flank and pelvic pain was the only presenting feature with no renal dysfunction. Early and significant decompression of left renal vein (LRV) via left ovarian vein resulted in preserved renal function with symptomatic pelvic varices. Endovascular management by left ovarian vein coiling and LRV stenting was done. They briefly review the etiopathology, imaging, treatment rationale, and management options for nutcracker and PCS.

Pulmonary Function Changes Before and After Mitral Valve Surgery in Severe Mitral Stenosis

Objective This study was undertaken to assess the pulmonary functions in the patients with predominant severe mitral stenosis before corrective mitral valve surgery, then to reassess the same 6 months after surgery and compare them. Patients and Methods Fifty consecutive patients with predominant severe mitral stenosis undergoing mitral valve replacement surgery were included in this prospective observational study. This study was conducted from July 2016 till January 2018 after obtaining approval from the institute's ethics committee and written consent from all the participants. All the patients were evaluated clinically and divided according to New York Heart Association (NYHA) class symptomatology. Computed spirometric pulmonary function tests (PFTs) such as forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), FEV1/FVC ratio, forced expiratory flow (FEF) 25–75%, peak expiratory flow rate (PEFR), and maximum voluntary ventilation (MVV) were performed on each patient 1 day before and 6 months after surgery. Results Fifty study patients were analyzed, mean age was 38.4 ± 10.76 years (15–56 years), and male-to-female ratio was 1:1.38 (21 males and 29 females). NYHA class symptoms improved significantly in most of our patients after surgery (p Conclusion This study showed that the lung functions are impaired in patients with severe mitral stenosis and improved significantly 6 months after surgery, which does not correlate well with the betterment of NYHA class.

Ectopia Cordis: Two Different Cases with Literature Review

Ectopia cordis is a rare congenital anomaly with thoracic exteriorization of the heart. With few exceptions, it is universally fatal. Surgical correction is the only final treatment option for this defect but with a very high mortality. The authors present two cases of ectopia cordis, both were operated upon in urgent basis, and one baby survived.
