OBJECTIVEDiagnostic and interventional cardiac catheterization procedures for congenital heart diseases (CHD) are becoming increasingly more popular, and arrhythmia is a well-known complication. This study was an evaluation of the incidence and causative agents of arrhythmia and the subsequent treatment strategies applied during cardiac catheterization.METHODSThe catheterization data of all of the patients who underwent diagnostic cardiac catheterization for CHD between January 2012 and 2018 at a single center were examined retrospectively.RESULTSA total of 1316 children underwent diagnostic cardiac catheterization due to CHD. The median age and body weight was 18 months (6 days-21 years) and 9.9 kg (2.2-135 kg), respectively. Patients with ventricular septal defect (281 patients) and those with tetralogy of Fallot (257 patients) represented 2 major groups of the study population. In 93 (7%) patients, arrhythmia developed during cardiac catheterization. Among them, there were 58 (62%) cases of bradyarrhythmia and 35 (38%) cases of tachyarrhythmia. Arrhythmia was classified as low, high, or major, according to the adverse event severity score; the rates were 2.7%, 4.3%, and 1.2%, respectively. In 36 (39%) patients, there was no need for therapy, whereas 57 (61%) required treatment to eliminate the arrhythmia. Treatment modalities included catheter manipulation in 15, pharmacological therapy in 24, and cardioversion in 3 patients. Eleven patients required cardiopulmonary resuscitation. Temporary pacemaker implantation was required in 2 patients, while 2 others underwent permanent pacemaker implantation secondary to catheterization-related arrhythmia. There were no cases of mortality secondary to catheterization-related arrhythmia.CONCLUSIONDiagnostic cardiac catheterization in CHD may result in various types of cardiac arrhythmias. The proper management of arrhythmias may reduce morbidity and mortality related to cardiac catheterization.

Aim: The aim was to evaluate the effects of antioxidant application and delayed cementation on the bond strength of fiber posts after intracoronal bleaching. Methodology: Fifty-five maxillary central incisors were used. Root canals were enlarged using Reciproc system up to R40 instrument. Root canals were irrigated, dried with paper-points, and obturated with gutta-percha and a sealer. In 44 teeth, root canal fillings were removed 2-mm coronally and canal orificies were sealed with glass-ionomer-cement. A 37% CP gel was used to bleach the teeth. Five experimental groups were defined (n=11). G1: No bleaching, G2: Immediate FPC, G3: Immediate FPC after SA application, G4: 14-days delayed FPC, G5: 14-days delayed FPC after SA application. Push-out tests were performed. Statistical interpretations were made (α=0.05). Results: There was no significant difference among the groups in apical thirds (p>0.05). However, significant differences were detected among the groups in middle and coronal thirds (p<0.05). Conclusion: The 14-days delayed cementation seems to be a more reliable method than the SA application before FPC procedure after intracoronal bleaching. Bulgular: Allojenik HKHT yapılan 46 hastada tekli infeksiyon veya koinfeksiyon şeklinde olmak üzere %26 (n= 12)’sında CMV, %26 (n= 12)’sında EBV ve %2.1 (n= 1)’inde ADV viral yükü 1000 kopya/mL’nin üstünde bulunmuştur. Allojenik HKHT alıcısı 20 (%43.4) hastaya preemptif tedavi uygulanmış ve 19’unda viremi temizlenmiştir. Sadece 1 (%2.1) hasta EBV infeksiyonu ve akut graft versus host hastalığı (GVHH) sebebiyle kaybedilmiştir.

Giriş: Aspergillus cinsi küf mantarlarının klinik örneklerde kültürde üretilme başarısı %50’yi geçmemektedir. Nötropenik ateş dönemindeki hastaların bir çoğunda ampirik antifungal tedavi başlanması nedeniyle, kültürde herhangi bir mantarın üretilebilme olasılığı azalmaktadır. Moleküler yöntemlerde hedeflenen mikroorganizmanın DNA’sı gösterildiği için tedavi başlanmış olması tanıyı olumsuz yönde etkilememektedir. Bu çalışmada, Aspergillus cinsi mantarların DNA’sının gösterilmesi için geliştirilen kalitatif gerçek zamanlı polimeraz zincir reaksiyonu yöntemi sunulmuştur.
